Recently I was performing some rooting trials with in-vitro produced walnut plants, at this time a lot of those plants have died from fungal infection.
I mention some background about:
- Induction in DKW 1/2 for 5 days in darkness
-Rooting in humidity chamber ~ 90%
- Peat-Perlite Substrate 1: 1 and Jiffy 7C
-In in-vitro conditions some of these plants had a white halo in the callus.
-Substrate humidity controlled
In previous tests I never saw anything like this.
The reason is it may be attacking of fungi, bacteria, especially the roots because of the remnants of media in the plants especially in the roots , during acclimation stage, Pesticide sterilization process ,the pesticide may be ineffective or that the low concentration or the period immersion of plants in the pesticide is short , it is not sufficient to eliminate it. You try with in vitro add in the medium charcoal and remove from darkness. Only provide the normal light 1000-2000 lux light provide. It is not fungal contamination. i hope rooting system development more better in the 60-70 % humidity. Excess of humidity plant under the stress condition and plant not proper nutritious not take from out side to inside. Finally root system is not proper growth and finally dead of the root system. you try with one set.

Its very panic to see contamination at the time of ex vitro rooting and hardening, I can understand this. As per yours query and explanation, it evident that these fungi were likely to come from the jiffy substrate or plantlets transfer error. The images posted by you revealed an initial attachment and proliferation of fungal bodies mostly with base of plants, thus I suggest you that kind of contamination. further, yours previous lot was free of contamination thus excluded other chances of endophytic fungi and explant or medium contamination problem.
It may be possible that the jiffy pots were old, so, sterilized new pots can be tested, and keep an eye during plant transfer into it. you may also sterilized the chamber you are using for rooting for precaution.

I agree with Dr.Antia, you must sterilize the pots, there are many fungal infect rooting like Phytophthora, Pythium, Fusarium, and Rhizoctonia

So it would be better to make the acclimatization by individual plant (small jar with bag close)
