Need to compare with pt's clinical history and signs . According to its lab parameter , there would be many differential diagnosis . Can't say exactly the definite diagnosis how much it is high or low. Rule out CRF .
In order to interpret these data much more details are needed.
For example: gender and age of patiënt, if female pregnant or not.
What do you call high? For example an incidental 10 % elevation of CRP compared to upper level of normal does not mean a thing.
What is an elevated ESR, give the data and if possible the lab's upper or lower level of normal.
How was the sample obtained, how long and under what conditions stored before analysis.
Was there a certain suspicion of disease or was it a routine check-up?
What other tests were done (chemistry tests?), which were normal?

I agree with above. Association of raised inflammatory markers with lymphopenia and renal failure is worrying and needs assessment by an experienced physician. Need to consider, infection, inflammatory diseases and malignancy.

Most likely a non-immune inflammatory state is going on. But then again nothing should be looked at in isolation; the patient's history and clinical picture, along with other tests, would narrow down the list of possible diagnoses.
Need clinical details and exact lab values. A chronic infection e. g. Sub acute bacterial endocarditis, melioidosis, brucellosis, even TB, etc, should be ruled out.
Complete history with clinical details required for reaching a diagnosis. Even these lab findings are inadequate. How much low or high is the value outside the normal parameters. And what about TLC and neutrophil count. And also platelet count?? Complete CBC with reference range of the lab should be provided
