I wrote a paper for a conference in 2012.  Then, early this year, I carried out a second research, improving my methodology.  I reviewed more papers, and improved my review.  Certainly we do not want to be accused of putting material from previous papers into a new paper. We want to be ethical.  How much should we paraphrase ourselves to avoid self-plagiarism?  

What can researchers do to avoid self-plagiarism? What are the consequences since self-plagiarism was highlighted by APA 6th edition?
