What are the main requirements to make a questionnaire to measure text readability and acceptabilty ?

In order to measure the readablity of translated texts from English into ARABIC, I want to prepare a questionnaire and to distribute it. Are there any special requirements we have to provide or a simple questionnaire is enough.
