Metformine (M) treatment alone or in combination with OCP is associated with weight loss and improved body composition compared with OCP, whereas free testosterone levels decreased during M+OCP or OCP. A randomized, placebo-controlled, double-blind control study in adolescents with PCOS showed that metformin significantly increased the likelihood of menses 2.5-fold, lowered total testosterone concentrations by 22 percent, and improved high-density lipoprotein cholesterol levels by 15 percent, although it did not significantly affect fasting measures of insulin sensitivity or body weight(1)
When metformin and OCPs are used in combination, the positive effects of metformin on high-density lipoprotein levels are counterbalanced by the negative effects of OCPs on these lipids(2)

Studies show metformin’s effectiveness in improving metabolic parameters and menstrual irregularities (PMID: 21302921), and OCPs’ efficacy in managing PCOS symptoms like hirsutism and acne (PMID: 18375013). These treatments can be used alone or together, depending on individual symptoms and fertility goals.
