Its depend on the age:-

Age 8 to 24 months, (a.) Profound hearing loss b.) Limited benefit from binaural amplification.

Age 2 to 17 years: a.) Severe to profound hearing loss b)Limited benefit from Binaural amplification . c.) MLNT and LNT less than equal to 30%

Age More than 18 years: a.) Moderate to profound hearing loss ..b.) Limited benefit from amplification defined by preoperative test score less than or equal to 50% sentence recognition score in the ear to be implanted and less than equal to 60% in the opposite ear or binaurally.
