Minor procedures like extraction: if child i give ketamine,if adult i give small dose fentanyl with midazolam,then the dentist will give local anesthesia and everything will be fine. That's my own practice i would like to share with you.

In my opinion it is irrational to combine propofol with benzodiazepines, I would choose propofol and opioid (ex remifentanil).

I think it is rational to pump low doses of propofol combined  with local anesthesia.

One or the other (or Ketamine), but not both. Propofol wears off much quicker.

I would suggest propofol only  intermittently ,because dentist will infiltrate the field with local anaesthetic.
