Should fetal middle cerebral artery blood flow be assessed if the umbilical artery S/D is normal in FGR fetus? Is the same for early onset FGR or late onset FGR?
No need to assess blood flow in MCA routinely , if you see normal haemodynamics in umbilical artery, if you do not suspect Rh-isoimmunization.

in late-onset small-for-gestational age Oros D showed that the degree of placental disease in late-onset FGR cases is mild, thus UA Doppler is normal in virtually all cases, despite normal UA PI Doppler, there is a high association with abnormal CPR values. So I think MCA should be assessed both in late-onset FGR with abnormal umbilical artery and in late-onset FGR with normal umbilical artery.
