Overview of the Belgian Healthcare System
Type of Healthcare System: Belgium has a universal healthcare system that is primarily publicly funded through social security contributions and compulsory health insurance.

There is still a pay out of pocket cost. When hospitalized, costs are partially covered. Therefore a lot of citizens have an additional coverage, often offered via the employer. Then all costs are covered when hospitalized.
Seeing a Doctor: If you need to see a doctor in Belgium, you can make an appointment with a general practitioner (GP) or walk into a medical center. For emergencies, you can visit the emergency room (ER) or call a duty doctor.
Billing and Payment: When you visit a doctor or hospital, you typically pay an upfront fee, which is then reimbursed partially or fully by your health insurance fund.
Visiting a GP: If you have a registered medical file by your GP, costs are immediately arranged with the health insurance, and then you only pay the out of pocket cost, like 4€,
If you do not have a registered file: you will pay like 23€, take the note from the doctor to bring it to the health insurance, and they will pay you 19€ (more or less) back. It is the same OOP payment,
So when you pay, you will get a prove for the health insurance, or it is possible that the doctor is connected to the insurance system, and then the prove is send automatically to the health insurance. Then you don't have to go to the health insurance to give the note. They will reimburse a part of the bill on your account.
If you have an appointment in the hospital, the bill is send later on via the post. People who are not paying their bills, or not on time, will pay 25€ in the hospital, to be able to go to your appointment, and then this is calculated in the billing. You can always ask the administration of the hospital to pay the bill in parts, if needed.
Access to Healthcare Services: Healthcare access in Belgium is generally good, but there are disparities based on socio-economic status
While the system aims to provide equal access, financial barriers can affect the poorest segments of the population.
This is normally not really an issue, as bills from the hospital are send to your home. And mostly the poorest segments of the population do have a beneficial statute by the health insurance, and then almost all costs are completely covered.
What I see as an issue is the dentist. There you have to pay the whole bill, wich is mostly several 100ts of euros, and not everybody has this. Of course, you will get a part back from the insurance, but you need to have it upfront. They do not have an agreement with the health insurances that you are able to pay only the OOP cost. You need to pay full price.
Elective Procedures: Elective procedures are handled based on clinical necessity and efficiency
Decisions on what is elective are made by healthcare providers, and these procedures are typically covered by health insurance if deemed necessary.

- I think we have a good healthcare system, we can choose who we visit without a referral note from your GP like it is in Germany, and in hospital is no difference made in if you are rich or poor. You will get the same treatment.

Potential Changes: Improving accessibility and reducing financial barriers could enhance the system. What I mean is it would be great if health insurances would have an agreement with all physicians, that patients only need to pay for the OOP payment. So that the costs that are covered by the insurance are immediately arranged between them and the physician. Then more people would be able to get proper treatment in stead of procrastinating because of financial issues.
Governing Body: The healthcare system is overseen by the Federal Public Service for Health, Food Chain Safety and Environment (FPS Health) Called RIZIV.
Dental and Vision Services: Dental services are partially covered by public health insurance. Advanced treatments may require additional private insurance. These dental insurances are often offered by the health insurance. You pay then a certain amount per year (I believe it is 220€ year for me and my family) and yearly they cover around 1700€., for each member.
For vision services, I don't think an additional insurance exist.
Your health insurance could offer reimbursement/ partial reimbursement. To give an example of mine:
Eyewear
For glasses (lenses, frames, sunglasses), The health insurance gives you a reimbursement of 25 euros as well as a 50% discount up to 50 euros at a participating eyewear shop. In addition, the statutory health insurance reimburses part of your costs in certain cases..
Lenses
The health insurance gives you a 25 euro refund as well as a 50-50 discount for lenses at a participating optician. In addition, the legal health insurance reimburses special lenses and contact lenses for medical reasons.
Lasering or lentil implant
For lasering your eyes or a lens implant for refractive disorders, the health insurance will give you a one-off reimbursement of 150 euros per treated eye.
