She has very severe venous insufficiency (C4 disease) and likely needs evaluation for possible endovenous laser ablation. This should be done after her pregnancy. In addition her deep veins should be evaluated for any compression syndromes such as May Thurner syndrome. Stockings and leg elevation is about all that can be done at this time. Agree that she needs a thorough venous duplex and potentially additional imaging of the iliac system. Severe, asymmetrical left sided disease is concerning for compressive/occlusive disease in the pelvis, most commonly anatomic iliac vein compression or May-Thurner. Most treatments would be deferred until after delivery, but diagnostics need not be, and may guide postpartum intervention. Compression helpful for remainder of this pregnancy- a number of configurations available that may work better, especially late term, than full panty hose style.
OMG! I agree. With left leg you need a rule-out of May-Thurner syndrome. Emergency Duplex evaluation and pls put a high compression stocking on. If it is MTS, recanalization + stent.
If no MTS, you have to search a rear animal. Genetic chimera or something else in that branch.
This is unilateral C4 venous insufficiency. Stockings and elevation are the best treatment modality during the pregnancy. She should be investigated about Pelvic congestion syndrome, arteriovenous fistula or AV malformation. Firstly dopler must be done, then venography can be done for definite diagnosis.
