A patient with leprosy can develop renal diseases like acute and chronic glomerulonephritis, interstitial nephritis, secondary amyloidosis and pyelonephritis. Why these renal diseases develops? Is it due to direct effect of lepra bacilli or immunologically mediated phenomena?

The clinical presentation is variable and is determined by the host immunologic system reaction to the bacilli. During the course of the disease, there are the so-called reactional states, in which the immune system reacts against the bacilli, exacerbating the clinical manifestations. Different renal lesions have been described in leprosy, including acute and chronic glomerulonephritis, interstitial nephritis, secondary amyloidosis, and pyelonephritis. It is an interesting topic, but back in Sri Lanka, I am about 30 years late for the research. Leprosy is almost eradicated in SL by now, an we rarely see renal involvement, let alone a lepromatous case. The new challenge is Leishmanisis.
