I'm trying to find a questionnaire that can measure the satisfaction rate of a company according the recruitment services that a Recruitment Agency provided.

I suggest designing the survey from the basics. First, research question (it looks ready), collecting earlier data (it is ongoing as I see). Then, define the population of interest, sample, data collection tool.
For designing the questions we need to identify indicators, KPIs what we want to measure (e.g., cost and time of recruitment for skilled jobs, managerial jobs, the first year performance of new hires, etc).
I do not have much idea as most of the time it is done in
house to ascertain efficacy of the activity. However in addition to Mr. Radics observations- design parameters/goals- requires clarity i.e. research relates to sector/segment requiring research. it varies from factories,industries,IT sector ,research, and even teaching assignments.
Various methods are on the anvil and as usual our peers and seniors do give guidance on studies on the fields & methods apt for the study. Specific KPIs for study has to be identified/selected-focus on the sample(sub set of Universe) for study ,in terms of hypothesis & objectives of research. through instruments & finally to indicate scope for further research while concluding the study. It is no doubt a challenging assignment as focus is on the recruiting organisation too. All the best
