The Childbirth Attitudes Questionnaire was validated in Farsi. it is recommended to use cut off value=28 for identifying fear of childbirth. I could not find any references indicating so. It is consisted of 14 items. Each item is scored 1–4 (1=not at all, 4=very much) with total points of 14–56.
I got the attached article which includes some psychometrics on Childbirth Attitudes Questionnaire. I also noticed it is 16 items in the article not 14. Mostly it is recommended to deal with the total scores of the scale rather than cut point {the 50% of the possible total}. This will give you a higher level of measurement: Ratio rather than nominal or ordinal
Why not measure the positive power to birth instead of focusing on negative fear of birthing?

In recent decades, researchers focused on childbirth fear, mothers' rights to choose painless childbirth or Cesarean, different methods to decrease childbirth pain, different instruments to measure childbirth fear, and so on. In comparison, in the beginning of the past century, mothers gave birth at home and childbirth was considered as a natural event. There was not so complaining of severe childbirth pain and fear. I sometimes think of doing a qualitative study entitled: "childbirth fear from the past to the present" to investigate how childbirth fear was fashioned. I remember 30 years ago, most mothers did not think of childbirth pain or request for cesarean in Iran. As the rate of cesarean was increasing, childbirth pain and fear were becoming a problem.
The more you give it attention, the more you make it valid. You become part of the problem instead of the solution to the problem.
From the findings of my research women are talking to each other about severe morbidities after traumatic vaginal deliveries and becoming 'fearful' of lifestyle consequences like sexual dysfunction, urinary and fecal incontinence, pelvic organ prolapse. This appears to be the more pertinent reason they are requesting caesarean sections. Women who sustained injuries like levator ani avulsion or OASI noted that they would have preferred information and informed consent prior to deliveries that resulted in unexpected and unexplained long term physical and psychological repercussions. Whilst vaginal deliveries can have positive outcomes for some women it is integral that clinicians be aware others suffer severe damage and up until recently have not had a voice and been dismissed as having 'women's problems that will get better'. Yes the paradigm has changed but not due to the fact women are less resilient.
