Even mild pre-operative anaemia is associated with higher post operative morbidity and mortality. In my experience - most surgical pre-operative anaemia is due to iron deficiency. Is it more likely that the multiple consequences of the underlying haematinic deficiency is responsible for the associated morbidity rather than any effect from the reduced Haemoglobin?

The question is very important as iron deficiency without anaemia should be investigated by iron studies and treated before all major surgery if it is the haematinic deficiency that impairs outcome.
