Female of 34 yo
Mother diceased at 35 with brain M1 of breast primary.
Breast lump study and biopsy, aglomerate of 5 intrammamary lymph nodes. Rest of breast negative

I don't understand. Where was the carcinoma? The 5 lymph nodes...did they contain carcinoma? If so, was there no primary site?

The 5 nodes where invaded by a triple negative, probably breast, carcinoma, but no other breast tumor/deposit found.
Was the tumour checked for BRCA? If positive would make breast more likely.
Did the lymph nodes contain lots of lymphocytes or mainly tumour cells? If lots of lymphocytes perhaps there was a significant immunologic response to breast tumour which has obliterated the primary.
Were the tumour cells subjected to IHC staining to give some clue as to origin?

It is a curious case...
You can consider too the first way of the blood's diffusion of the breast cancer , that is the brain.
I would not call her M0 until you show that the beast cancer is not in itself a metastasis from another area. That is why IHC staining might be of help in locating a primary. It is likely breast, but there is no proof of that at the moment.
