I usually perform distalization of tibial tubercle as the first procedure to correct the patella alta. If need be, I add MPFL reconstruction as the second staged procedure. I never had to add trochlear plasty.

I do agree with Dr EDELSTEIN. FULKERSON procedure might be enough and if needed secondary MPFL reconstruction...
I do have considerable experience with trochleaplasty but feel that it is not indicated in grade 1dysplasia. I therefore agree with a combined TT Transfer with MPFL Reconstruction in this patient.

Static image exams are always important in patella instability, but I will never decide which surgery will be more appropriated without evaluating the patella tracking during active flexo-extension. In patients with subluxation in extension, Fulkerson alone is frequently not enough.

If you have IP = 1.34 is required distalize TT for next index of 1.0 and transfer TT to 11-12mm.
Following the school of Lyon, another important fact is the patellar tilt. If it is above 20 degrees, should be done MPFL reconstruction.
I would not do the trochleoplasty because the degree of dysplasia is small..
