I would like to understand if an Monty Carlo simulation is made for an External beam radiotherapy system (6 MeV) only with primary and secondary collimators, will the flatness vary because of not using the applicator during testing and actual Monty Carlo simulations.
For a photon beam no, for an electron beam of course since the focusing effect of the electron applicator will be missed. If you remove the applicator for measurement and Monte Carlo simulation you should get comparable results.

Electron applicators fill up the edge region of the eletron profiles by electrons scattered in the air volumes outside the geometrical shape of the radiation fields. I add a chapter out of one of my textbooks where you can get an impression of this effect. See chapter 9.3.3.

the applicators plays an important role during the electron beam therapy because they flattening the electron beam around of treatment field size and without applicator the beam profile get a cone shape.
A single foil produce a dose profile with the high intensity profile ,but further scattering from the air and wall of the solid applicator produce a beam of more uniform intensity.although this amount of scattering from air and applicator is not enough ,so we use double foils.
So its role is flattening more and collimate beam close to the patient.
