If all reagents are fresh, make sure all glassware is clean, an electrophoretic chamber, gel casting glass plates, and a comb are well washed, all plastic (tubes, tips, pipettes, weighing dishes, etc.) are not re-used. Good luck!

Can you label which wells you actually loaded the samples into?
