Although its difficult to describe lesion with certainty as it is not so clear but it looks to me as Atrophic mottled pigmented macular lesions in cluster at dorsal aspects of feet. ? Poikilodermatous syndrome?
I don't think it's purpura but more of a hyperpigmentation spots like lentigo or freckles. Although you can test it by give some pressure on it or use dermoscopy to help the visualization.
Probabile Angiodermite pigmentaria purpurica di Favre Chaix. Indagare sulla eventuale presenza di insufficienza venosa , sulla assunzione di farmaci come aspirinetta od antiaggreganti, sulla eventuale presenza di carenza vit C.
The rash shows mottled brown pigmentation - from my experience I have seen this presentation secondary to pigmented purpuric dermatosis, capillaritis and rarely contact dermatitis.
