Dear all,

I am busy working on the documentation for my annual appraisal. Part of my role is the supervision of trainees/students in projects that will often result in the presentation at a speciality conference and also sometimes in the publication of an article. Probably procrastinating here, but whilst this issue is too late for my upcoming appraisal, are there any recognised formal questionnaires to receive feedback for such supervision?
