Suggest the following:
Perform thorough literature review on the constructs / variables in your conceptual framework / research model you want to measure via questionnaire.
Determine the operational definition for each construct / variable
Perform thorough literature review on questionnaire to measure those constructs / variables including their previous reliability & validity scores (sometimes the literature where you obtained the operational definition will provide some questionnaire).
Adopt & adapt the questionnaire according to your research context. If no questionnaire meet the expectation, you can based on the construct's operational definition to develop your own questionnaire (follow the good practice in developing your own questionnaire - you can find these good practices from typical research methodology books e.g. simple & concise language, no multi-barrel question, formative / reflective questions etc.)
If your questionnaire & operational definition derived from different literature sources, please review & ensure the questionnaire measure all the components of the operational definition of each construct.
Review the questionnaire you developed on your own with subject matter expert to ensure they are valid.
Perform proper translation & verification of the questionnaire from e.g. English to local language if needed (also get language expert to translate from local language back to English to see anything missing or misinterpreted).
Conduct a pilot study on your questionnaire with some respondents to fill them up - guide them how to fill up & fix any error hilighted by the respondents e.g. spelling & grammar errors, choice of words for better meaning etc.
Perform reliability test like Cronbach Alpha / Composite Reliability & some validity tests to ensure the questionnaire meets minimal requirement.
Perform more pilot study if needed. If above are done successfully, you can use your questionnaire for actual data collection for all the constructs.
This is a complex issue that would require more discussion than this RG can provide. I suggest you read some of the literature on this, and one good introductory book is Fowler, Survey Research Methods (Sage).
1. In my opinion, before thinking of "how to make questionnaire", it is way more important to decide "what I want to study" , what variables/constructs will be involved and whether causation is of the interest or the correlation.
2. Once variables are identified and what kind of analysis is involved, we look into "how to measure the variables". This is where we decide "what type of questions will be asked",
And David has suggested a very good book in this regard and kind of covers everything I mentioned above.
