Good question.
People test twins' genome makeups to find out. If identical twins, their genome makeups should be identical. So, if there is no any body tissues left for molecular testing, I don't know whether there is another way can be used to distinguish them.

So, even before they were born, one still have a chance to tell whether they are identical twins if ultrasound was taken.
