Are there any self-report questionnaires designed to assess a patient's individual model regarding his (mental) disorder? For example, how a person rates the contribution of biological, social or psychological factors to the development and maybe even the treatment requirements of their disorder.
At the moment, I am only aware of interviews (e.g., OPD 2) that include such an assessment.
