As per the manufacturers of Dexmedetomidine (Precedex Concentrate for infusion), the use of therapeutic dose of Precedx for sedation of initially intubated patients during treatment in an intensive care setting as a continuous infusion should not exceed 24 hours.
How long do you really use this drug?
What is your justifications if you are using any more than 24 hours.
In the pediatric ICU settings, it is used sometimes for several days and titrated to sedation level, while observing for potential side effects.

I would add that adding this medication to our sedation protocol was very helpful, especially for children and infants on Non Invasive Ventilation (NIV).

The Dexmed starting dose 0.3, then titrated as per the COMFORT score
up to 1 mcg/kg/hr (100 mcg in 50 ml NS solution)
Up to 7-10 days in infants on NIV
If there is any side effect (especially Brady): decrease or replace with alternative
