Psychologists have a much bigger role to play with increasing day to day life stress. Do we tend to pile on the stress and avoid seeking the right advice?
The most important in problem soving approach in health care is find the cause, not only treat the symptomes. Pain may be caused by stress, but can we use only medications to treat pain?
Dear colleagues, One of these days I read the article - Treatment of central sensitization in a patient 'with' unexplained 'chronic pain: an update - (.. Nijs et al, Export Parmacother Opin 2014; 15 (12): 13). An overview of treatment options for desentization in patients with chronic pain is provided. A combination of different strategies - each targeting a different desensitizing mechanism - might prove superior to monotherapies. The question of colleague Emilia Mikolajewska - Pain may be caused by stress, but can we use only medications to treat pain? - is a very impotant question in relationship to the central question of colleague Ankush Sachdeva. Besides pharmacotherapy, conservative treatments - for instance pain neuroscience education, cognitive behavioral therapy and exercise therapy - are promising options for pain based on central sensitization. All these treatments options are within the competence of physiotherapists familiar with actual insight into pain neuroscience and pain mechanisms of central sensitization. It is advocated to combine a bottom-up approach (peripheral to central to decrease peripheral nociceptive input like partial immobilisation of peripheral joint combined with muscle strengthening exercises) with a top-down approach (central to peripheral like pain neuroscience education combined with functional exercise therapy of daily activities). My considerations in the question - Do we tend to pile on the stress and avoid seeking the right advice? - are that we as physiotherapists do not pile on the stress and we can give the right advice and treatment based on modern pain neuroscience, mechanisms of (chronic) pain and central sensitization, clinical reasoning and best available evidence. Although I am not an expert in the field, but I will say that treating such kinds of ailments need:
1. Suppress the symptom and stabilize the patient through medication and professional counseling
2. Investigate and find causes of the phenomenon/symptom
3. Let the cause be known to the patient and at the same time to inform and remind the patient constantly how innately powerful our brain is to create, bend, deviate, deform and magnify or nullify and minimize truth and become the most dangerous weapon of self destruction or self promotion and delusion.
The combination of these actions will help the patient to be a self diagnosing and remedying person at the same time in order to lead a perfect life.
Stress related issues counselling is very important. But at the same time one should know where to draw the fine line with counselling and medical therapy. For example in stress cardiomyopathy. The extreme stress causing heart failure which needs to be diagnosed and treated medically where counselling has minimum effect.
Regular counseling is quite important and better than medicine in stress related issues as it give a kind of relief and one fells lighter after having counseling.
I agree @ Prof. Meena. In general, Yes but cause of stress only determine it will be cure by counselling or medicine. In some cases, it will be reduced by both.
The major problem is to determine the cause, nature, caracteritics (severity, recent or not,etc.) of the stress.
If it is possible to help people by councelling: it's very good...Unfortunatly in many complex situations medecines must be associated with councelling to allow it some efficiency. In other cases (mental illness) medecines must be used first and councelling or psychotherapy second when patient is stressless by the action of medications . The fondametal work of councelling can be effective to help people to control stressors, becoming more relax and preventing relapse to stress.
Handling stress, is a individual control ...many relaxation skills such as music, meditation, diaphragmatic breathing exercises, aerobic exercise, muscle relaxation and imagery etc.,
Does anyone have some examples of such situations. I think biggest role is in domestic stress and after that patients in chronic illness or recovering from major surgery also feel depressed.
Counseling or medicine for stress..both can go side by side, However,one first go for counseling but we can not deny the role of medicine. Every individual case is unique, its difficult to generalize. A mental health professional can better decide from where to start and what to start.
Medicine only cures 50% as the doctor counsels when Dr. is prescribing Drugs... For example if the doctor does not say words like 'take this antibiotic course for a week you will be alright'... If he does not do this counselling, think I have given you drugs based on symptoms, let us wait and see how it gets acting on you... may or may not be effective, if he says like this shall we ever go to him...
In my decades of Critical medical device experience, the best doctors do best counselling along with excellent diagnosis.
Whether counselling alone will cure? YES, but it is a very long process of natural healing by body metabolism... Most of the time we do 'animal study' for any medical products... For example, we are placing the foreign body 'Metal Stent' in heart and other places... body does not rejects it, but accommodates it.
Many animal injuries are natural healing process... so is the case with us too but will take very long and one need to have lot of patience as a patient... In a rushing world we do not have patience, hence quick heal is medicines, surgery...

patients and attendants feel more comfortable when the doctor keeps on counseling them in between during illness period. The time spended with them always decides the attitute towards you....we need to involve the psycologist more often...
Counselling process is interactive and the patient feels comfortable . It is not information giving but a process learning about the patient and giving the options to the patients to decide and then act. The responsibility of the Counsellor is to assess the needs of the patients seek the possible situations /risks and then give options. Comprehension is an important part of the process.
When a doctor gives a advice to the patient, keeping the patients medical condition in mind this also works well for the patient, but that is a medical advice.
For a patient both medical advice from Doctor and the counselling where options/choices to change for long term diseases are important.
Needless to say, use of counseling or of meds depends on individual cases. I know of some patients who regard counseling as intrusion, Others cannot find counselors to suit their particular problems or temperaments. A bond of professional friendship should ideally arise between counselor and patient. If this is missing or if in rural communities professional counselors are not to be found, a trusted primary care physician can administer advice and/or meds.
Dr Nelson has rightly said. not all view counseling as medicine. counseling need not be verbal, even body language is important. observation of the patient is more important.
chronic disorders like psoriasis, or a simple chronic itch can irritate a person. medicine in the least quantity can be a lot better. its more important to let him know you care
counseling today is being handled by psychologists. emotions are best understood by a doctor who knows the disease.

Mr. Sachdava: In general, I believe that medicine is aimed at abating symptoms and counseling is about empowering the patient to make decisions, assess their situation, and moving ahead. There are some exceptions: for example, the patient with PTSD may need anxiety reducing medication for some time, while also being involved in talk therapy, whether individual or group. I'm afraid I do not agree with Dr. Cansani who states that depression is cured by a meaningful relationship. First, I would like to know how one defines "meaningful" and "relationship" and "depression." Individuals can work with their healthcare professionals to find a good, helpful balance between using chemicals (esp. because of all the research about mental illness and brain activity) and talking, focused, goal oriented. Stress is usually self-caused and there I would suggest some self-reflection about the causes of the stress and possible ways to relieve it without resorting to alcohol or street drugs. Best Wishes.
Agree with @ Mary that counseling supports decision making towards health. Actually Counseling allows a person to talk out their problems and feelings in a confidence and dependable environment. To gain a better understanding of the person's feelings and thought processes, as well as identifying ways of finding solutions to their problems. Suited to handle - depression , borderline personality disorder (BPD), obsessive compulsive disorder (OCD), eating disorders - anorexia
Imagine a situation where X is having a chest pain rushed to the hospital. Reports and examination suggest immediate action. should I call a councellor or should I take action.
2. follow up of the case regarding near death experience support and strength to live and to carry on with routine. the doctor knows when and how a next episode can strike. How will the councellor take up these two situations.
Yes, of course we are discussing a subset where interaction is important and is not thought of..its place is always after excluding medical causes for the same. Constant sessions of such talks keep people stress free and positive in difficult times and makes them comfortable.
I am in a full agreement with you: counselling is very important.
In some cases counselling support, psychotherapy, behavioural and cognitive therapies (many possibilities) alone can help support and healing ...in other cases they are essential complements that optimize medication treatments of stress, anxiety or depressive disorders... they reinforce efficacy and optimize the patient monitoring after cessation of medicines.
They reinforce patient selfconfidence, allow the patient to become himself free from pain and can help him learning the mechanism of stress or anxiety in the objective of more staying power. Finally they allow to recover faster in a better quality of life.
Yes, many a times the CT scan and MRI show that the disease process is over, but the person is not able to recover from the mental trauma and has lost his self confidence. In this situation one shouldn't feel that he is complaining unnecessary. One should take the help of counseling.
