I'm currently writing my thesis on general education teacher attitudes/behaviors when it comes to inclusion, specifically students with severe/intense needs. I was wondering if there were any survey/questionnaires I could request to use or be guided to some that would help in this. Or maybe someone has some suggestions on types of questions to ask.

you can make a careful review of the subject and go reviewing the background. There are different researches and researchers that provide their instruments, but if they come close to what you need, you must request the authorization of the author and make the adaptation to your population.
It depends on your methodology. If you are investigating this question from a social constructivist perspective, then you should derive about 10 open ended non-leading questions, that examine all aspects of the phenomenon, and informed from the literature. if you are investigating from a post-positivist stance, my suggestion is to exactly operationalise the variable you are interested in measuring, then do a database search for available surveys that have been developed for that variable (keeping in mind the psychometrics of the instrument).
