There are different publications available. Some can be downloaded for free. The problem of most of these methods is to get (high) good quality DNA. JQVE published an extraction method for FFPE samples. Check out this demonstration video on their website.

QIAgen kit provides higher yield of DNA. Promega kit provides higher qulity of DNA. If you need to do regular PCR or RQ PCR, QIAgen kit is good. If you need to do fragment analysis, Promega kit will be better.
