I would like to ask questions about cutaneous leishmaniasis:
1- Is the measurement for the lesion from the lesion itself or from the reddish borders, so we can determine intralesional treatment location?
2- I want to ask how the sand-fly is able to choose the lesion location to take blood and the parasite and what about visceral leishmaniasis. Whereas the parasites are not in the blood, it is in the spleen, liver?
3- What about the diabetic people with cutaneous leishmaniasis can we treat them with systemic glucantime?

Congratulations to Dr.Lakal by his "master class" on management of leishmaniasis.I have been working in a litoral city (Ubatuba) about 200 kilometers from S Paulo were cutaneous leishmaniasis occurs in breaks,one for about each 10 years.The last one we followed occuried about 10 y ago, total cases studied were about 60. Now, 10y after, we are waithing...
Severe cases are refered to Prof Lindoso, at Instituto de Medicina Tropical de S Paulo.

Dear , firstly I praised Sami and Joao Luiz's answers ; then secondly the amastigotes found the cytoplasm of the skin tissue fixed macrophage and other types of mononuclear phagocytic cells and R.E .system cells , so the sand fly could take these cells during feeding on blood host ; Thirdly the insects are attracted to the host skin or lesions by different factors such as Carbon dioxide ,body odor,secreations (saccharides and antigens) , Blood type , Lactic acid ; THanks
