Dear Professor Evens Emmanuel , in my viewpoint, at the same time, health systems must be strengthened, organized from the local existing meanwhile any international support and, the populations should be involved in the responses. Social representations of the disease should be questioned in order to sensitize the population about adequate health behaviors.
The dissemination of scientific information aiming to sensitize the population must consider its social perceptions. Thus, the responses will be adapted and, more easily, accepted by the population.
Dear Professor, it is important to point out the reasons for the limited investment in health systems and the current critical situation evidenced and augmented with the COVID-19 crisis in developing countries. Unfortunately, public policy and investment priorities in many developing countries do not target human capital formation (education, health). This is an obstacle for human development. In many poor countries, even military expenditure is bigger than health investment (as a % of GDP). Hopefully, the crisis would contribute to a change in government priorities toward stronger health systems. This is a necessary condition to move forward with robust response systems for national health emergencies. The case of El Salvador (in Central America) is an example of how health system investments can be implemented in a relative short period of time, to face the crisis and improve epidemiological management.

In order for epidemiological surveillance to be strengthened to prevent developing countries from avoiding carnage with Covid-19, the response of the authorities to epidemics must be coupled with high quality data and appropriate analysis. When an initially localized epidemic acquires pandemic status, this association becomes an essential element for ensuring coordination, preparation and control on a global scale.
However, access to rigorous analyzes becomes difficult during emergency situations, for example due to the weakness of health systems, obstacles to access to care, capacities to test, sometimes limited capacity to collect and data management or insufficient means to carry out the appropriate analyzes.
These constraints were identified during the major cholera epidemics in Haiti and Yemen in the context of the seventh pandemic. The cholera epidemics in these two countries have illustrated the difficulties in assessing the magnitude of the spread and mortality and their evolution due to obstacles to access to testing and health care. The implementation of appropriate public health interventions has been compromised.
In the context of the Covid-19 pandemic, many countries, including those that are generally reluctant to share data, have provided access to relevant clinical and epidemiological information.
However, differences in access to health care and laboratory tests are currently two of the main limitations that prevent a clearer picture of the Covid-19 pandemic.
