Initially, the individual appeared to be intellectually gifted and possibly on the autism spectrum. It has become evident that she whispers for an unknown reason, while others are around, without apparent realization that she is doing it. Occasionally, she will hold her hand in front of her mouth while whispering. It may be a sentence or a 30 second speech. One time, she spoke normally to a companion, then whispered, then spoke normally again. It happens more often when she is tired. Are you familiar with this phenomena? Are there any thoughts other than schizophrenia? Thank you.
Yes. I do remember such whispering in an older lady diagnosed with paranoid schizophrenia
I have a client, an older gentleman who regularly whispers, and at times sounds quite deep and intimidating, this whispering does not seem to have any affect on any other behaviour and can occur at any time of the day or sometimes night. Generally saying his name directly too him almost snaps him out of the whispering back to the attention of the person saying his name.

From all appearances it seems to be an internal dialogue that he just speaks out loud, there is not back and forth dialogue just whispering in varying degrees and seems overall fairly harmless in this particular individual. The individual I'm discussing has been doing this for quite a few years.
I observed this in a client who was suffering from schizophrenia but who was also on the autistic spectrum, particularly when reliving past experiences.
Hai. I observed this with a normal person when he is in difficulty and trying to solve a problem, he whispers to himself like talking to his inner-self in a "loud" for an answer.
