Management of hyponatremia depends on assessment of volume status which is clinical and prone to inter observer variation. Bioimpedence analysis by multifrequency analyzers accurately measures the fluid status in various body fluid compartments .
No for more than two reasons.
1- the conductivity does not change ( only 2% maximum) between 100 and 150 milliEqu ( this is the window of the " in vivo" changes)
2- Impedance is volume dependent, Sodium concentration not ( or not necessarily)
Terefore
3- Z-spectroscopy for ECW does not" measure accurately ", in any compartment
Finally it lacks of sensitivity and sepecificity with respect to Na.
It is time make everybody aware that the best "precison" that Z in ANY FORM can yield is "about 2 liters" or more of SD.
