All or none studies are frequently mentioned in levels of evidence tables, but I have yet to find an example of where one has been undertaken.
Very interesting question. If I am corret the definition of "all of none" is that all or none of a series of people (case series) with the risk factor(s) experience the outcome. The data should relate to an unselected or representative case series which provides an unbiased representation of the prognostic effect.
If I'm again correct the following study might be an example of all of none study. I guess other examples might found in vaccine studies.

CONTEXT: Understanding Medicaid performance relative to private payers and among other states may lead to better value.
DESIGN, SETTING, AND PARTICIPANTS: Hospital Quality Alliance data from 2007-2008 were used to create composite "all-or-none" quality scores for nearly 900,000 nonelderly adult patients hospitalized with acute myocardial infarction (AMI), congestive heart failure (CHF), or pneumonia.
MAIN OUTCOME MEASURES: Differences in the quality of care received by Medicaid compared with privately insured patients at the national and state levels.
RESULTS: Nationally, 88% of Medicaid beneficiaries received all appropriate
processes of care when hospitalized for AMI, compared with 73% for CHF and 77% for pneumonia. Private patients received higher quality of care than Medicaid patients, but differences were small (1.3 percentage point difference, pneumonia; 2.7, AMI; 2.9, CHF; all P

I think " All or None studies" are beyond case series. These could be earlier stage RCTs on human which are found to be efficacious but we have been looking for the intervention on the larger sample.
