Does adaptive control work for auto piloting the aircraft control? If it works well, then why swiss air 711 and TWA 810 met an accident in 1980s?

Adaptive Control and MIT Rule for Adaptive Control

One can imagine the whole control process for avoiding any accident as two loops. The outer loop which includes pilot, ATC and ... which should make the critical decisions about issues like the path and the inner loop which includes pilot, Autopilot , Aircraft and ... which are responsible about the safe following of the path. As I know the auto pilot 's job is not avoiding any conflict between aircrafts and its pilot's responsibility.
