Since 5 to 15 percent of patients with aortic stenosis under going TAVI are reported to have concomitent amyloidosis, i recommend screening for amyloidosis in such patients before going for TAVI. Echocardiography with 2D strain and MRI are the tests i would recommend for screening. To confirm amyloidosis in suspected cases bone scan are recommended.
Cardiac MRI with T1 mapping and ECV quantification will be the ideal investigation to rule out cardiac amyloidosis in patients with AS. ATTR amyloid may co-exist with AS in about 15% cases. Confirmation with Tc scan.
