In recent years the acceleration in the rate of chronic diseases that involve insulin resistance has become of major concern. Non alcoholic fatty liver disease (NAFLD) is expected to escalate to approx. 30-40% of the global population by the year 2050. Insulin therapy has been of critical interest to maintain glucose homeostasis and prevent mitophagy that has been a critical factor in NAFLD and multiple organ disease syndrome in diabetic individuals. The use of INDIAN SPICES such as curcumin and turmeric has been indicated in many communities and associated with glucose control in diabetics. Curcumin has been used as an antimicrobial in food and normal individuals may consume curcumin or other Indian spices in their food (taste/flavour) for several years before becoming DIABETIC.

The critical use of INSULIN THERAPY in diabetics is to maintain mitochondrial biogenesis and the use of eg. CURCUMIN (plasma concentration ???) in these diabetic individuals diet need to be assessed before dose and frequency of INSULIN is administered. Insulin therapy has been seeded as science/biology in MEDICINE as one of the most important discoveries that has maintained cell survival in many global communities for several years. The use of combined Insulin therapy (dose/frequency) and curcumin (dose/frequency) may lead to accelerated glucose entry into cells with unexpected hyperglycemic induced mitochondrial apoptosis associated with myocardial infarction and neuron apoptosis. Indian spices are not cleared from the body (turmeric/curcumin accumulate in the body).

Insulin therapy in diabetics when controlled by diabetologists allow the use of insulin to release from cells the anti-aging protein Sirtuin 1 (plasma concentrations). This protein is an acute phase protein and Sirtuin 1 is connected to the prevention of insulin resistance. Insulin therapy is critical in DIABETICS to reverse NAFLD. and prevent MYOCARDIAL INFARCTION.

Presence of bioactive compounds (polyphenol, flavonoid, and anti-oxidants) in different herbs and spices MAY help control chronic diseases such as Type 2 diabetes, cardiovascular and renal diseases. Since ancient time, consumption of herbs and spices were well known for its medicinal properties and India is ‘home to spices’ that are used prominently in traditional medicine practice. In type 2 diabetes, a metabolic disorder in which enough insulin is not produced/released to regulate blood sugar levels, the high level of sugar in blood further promotes inflammation. The anti-inflammatory effects of natural herbs and spices block the formation of inflammatory compounds.

Spices, Indian, Asian or otherwise tend to share common pro-health properties: nrf2-ARE upregulation of endogenous CAT, GSH, HO1, etc, activation of AMPK-Sirt1, mTOR inhibition, Nf-kB pathway inhibition, GLUT4 up-regulation, pancreatic lipase inhibition, etc -- where most of these pathways and effects overlap.

(As an aside, an emerging null hypothesis is that any polyphenol is likely to send at least some degree of impact toward the same list of benefits through the same pathways. If a pathway is more well known (ex: sulforaphane-nrf2 or curcumin-nF-kB) such is likely to result of which particular pathways the first studies were designed to look for in the first place.)

I would say the conservative standard of "May help control" is almost tautologically met, where "cure" is merely the far side of the impact distribution curve (more dependent on the presence of additional synergistic factors (allele type, diet type, activity levels)). On the time axis, I would regret any focus on any particular age or any multi-year duration. Rather, 3 months of improvement would impact at the A1C level. 48 hours of improvement would impact the plethora of benefits tied to partial depletion of hepatic glycogen stores. 2-3 weeks exposure to a different dietary regimen is enough for most proteomes to shift from sugar burning to fat burning.
