Do I need a Biosafety facility for just isolating DNA/RNA/microRNA from blood of a TB or cancer patient? Please note that I will not be culturing the cells.
Well in my own opinion, I would recommend that you do your extractions in a Biosafety facility. Please review this document. Ideally, TB being a communicable disease, pose a threat on the researcher. Bio-safety facility ensures the safety and prevents contamination as well. DNA/RNA/miRNA are molecular component of the cell and prone to external contamination as microbes can easily contaminate it. Cancer, although non-communicable, but impose threat on molecular extraction.

As per the international guideline, for handling TB samples, it require P3 facility if you don't know whether it is Drug Resistance case or not. ? At least Biosafety II is required because its Air born disease. As per the IBSC guidliine Biosafety is Must for handling TB samples in lab. You can refer "Guideline and Handbook for Institutional Biosafety Committees" .
Though it is not required to maintain aseptic condition for DNA extraction. As TB is communicable infectious agent, you should take some extra care for your own mental satisfaction. DNA of TB should be extracted at least in level 2 Bio-safety cabinet.
Tuberculosis is fairly common amongst Indians due to lack of appropriate care. Most Visa applications have a mandatory test for chest examination if you are traveling abroad, considering every third Indian is infected. It depends on your institution, and your state or country - the regulations vary! For example, apparently there are some things which are much more dangerous in California than in Texas. Contact your institution's occupational health or safety office and ask their advice and guidance.
Please be cautious and do your best to adhere to guidelines not necessarily with your institution but internationally verified. I say this because some institutions may be loose with their guidelines and the enforcement of it. I think one must be careful irrespective, when working with such type of specimens. My institution is pretty on top of things as regards safety in a lab environment (My lab adheres to guidelines strictly).
