CLINICAL CASE. WHAT WOULD BE THE RIGHT TREATMENT?

An interesting case of pain in the right side!

77 me woman with a past HX med of right retroperitoneal "hematoma" diagnosed 3 years previously presented w / a new pain CSD 8 months ago. A CT abd / pelvis showed a large CSD mass measuring 10.9 x 8.3 x 15.9cm and the participation of the abd wall musculature at the level of the 12th rib. The biopsy showed a de-differentiated liposarcoma. She received radiation to the mass 4-5 months ago. At follow-up 3 months ago, the mass had increased to 21.3 x 8.7 x 8.3 cm. In addition, several new enhancer areas of omental nodularity were prominent in the upper left quadrant. These results were worrisome for peritoneal sarcomatosis.
