Hello, if your drug not alkaline and not acidic so the dilution in distilled water will not change pH significantly.
The pH of the buffer solution remains approximately constant when diluted and added with acid and base. If you dissolve the drug in water with the addition of only acid or alkali, then when diluted with water (we wash down the medicine with water) the property of the drug may change.

The logic behind using a buffer is having a media with almost constant pH. That is the pH does not singficantly change in addition of a small amount of acid or base or on dilution.Moeover, in our body the pH of blood or other fluids have constant pH. So, it is preferred to dissolve drug in a solution with constant pH and any study of drug, such as drug delivery, should be performed in constant pH.
