I got ethical approval for my research, but while submission, I noticed that editors comments need changing one word to the title into another word. Can I change it while I got ethical approval with old title? Will the journal ever ask for the actual ethical approval document to compare titles?

No need to change the ethics. Editor comment only aim to improve the quality of your research.
Of course you can change, but you must check it's suitability as per your research work and the literature available.

Yes you can change a word, but then you need the following:
1. Revert to your Ethics Committee and re-submit with the new title (with the one word change you want to induct).
2. Then go to the trial registry site where you have registered your trial, submit the new ethics committee approval of your protocol with the changed title, and amend your title there too.
3. After that you need to submit your article.
The above is important for:
- Readers when looking at your article may want to go to the registry site to look at your study plan. If you do not amend the title in both the places (ethics committee, submission journal, and trial registry site), then the navigability of your work enlisted versus what got published will suffer; bringing a lowering of interest among the readers who are looking for true evidence.
I agree with Mahesh Kumar and Amitabh Dutta, "if the contextual meaning is changed with the word you changed it is better informing the
ethical committee". Maybe you can get the answer also in the documents of the ethical committee.
