Your question is not clear. The assessment of PHC depends on the settings. As the Health Systems are not similarly organized, tools to assess are also differents.

Check out the suite of patient, provider, and organizational surveys that we help develop in Canada. You can find them on the Canadian Institute for Health Information website.
