Emetic syndrome is caused by ingestion of a preformed heat-stable cyclic peptide toxin called cereulide, encoded by the cer gene. This syndrome has a short incubation period and recovery time. The symptoms of nausea, vomiting and abdominal cramping occur within 1–5 hours of ingestion, with recovery usually within 6–24 hours. Diarrheic syndrome is caused by heat-labile enterotoxins, encoded by hblA, entFM, and bceT genes, that are released by vegetative isolates present in the small intestine. The incubation period before onset of Diarrheic syndrome is 8–16 hours and the illness usually lasts for 12–14 hours, although it can continue for several days. Symptoms are usually mild with abdominal cramps, watery diarrhoea and nausea.

Hoda's answer gives some of the details, and it is important to note that it does take time for the bacteria to grow and produce the enterotoxin for the diarrheic form.

Bacillus cereus is a toxin-producing foodborne pathogen that causes two kinds of gastrointestinal disease: emetic (vomiting) syndrome and diarrhoeal syndrome. Vomiting occurs after consumption of a contaminated food when the emetic toxin (cereulide) is formed in the food. Diarrhoeal syndrome occurs when, after ingestion of food infected with B, enterotoxins are formed in the intestine.

Bacillus cereus can cause food poisoning in two forms: diarrheal and emetic. The diarrheal syndrome, usually associated with ingestion of meat or poultry, vegetables, and pastas, is characterized by an incubation period of 8 to 16 hours. The emetic toxin causes emesis (vomiting) only and its structure has for a long time been a mystery, as the only detection system involved living primates. The recent discovery that the toxin could be detected (vacuolation activity) by the use of HEp-2 cells, has led to its isolation and determination of its structure. Although there has been some doubt as to whether the emetic toxin and the vacuolating factor are the same components, there is no doubt that it is the same toxin. The emetic toxin is resistant to heat, pH, and proteolysis but is not antigenic. The biosynthetic pathway and mechanism of action of the emetic toxin still have to be elucidated, although it has recently been shown that it stimulates the vagus afferent through binding to the 5-HT3 receptor. The number of enterotoxins and their properties has also been debated for a long time, but at least three different enterotoxins have been characterized. The early studies on the enterotoxin suggested a single or a multicomponent enterotoxin. Further work has now shown that B. cereus produces two different three-component enterotoxins. A three-component hemolysin (HBL; consisting of three proteins: B, L1 and L2) with enterotoxin activity has been purified and characterized. This toxin also has dermonecrotic and vascular permeability activities and causes fluid accumulation in ligated rabbit ileal loops. HBL has therefore been suggested to be a primary virulence factor in B. cereus diarrhea.
