The resolution of DTI is generally not high enough to achieve monosynaptic fiber tracking in vivo. There is a paper of Bai et al with microscopic resolution (www.pnas.org/cgi/doi/10.1073/pnas.1519890113), which might be of interest for your question... Newer techniques using Kurtosis imaging might give you some more details when reconstructing fibers. But for all DTI based techniques one has to talk about "most probable fiber bundle tracks" due to the underlying physics, implying that there is only stochastic information about a synaptic connections. Where an axon ends is not answered.
For further reading I'd suggest some pre clinical studies of Harsan et al and some studies on fiber reconstructions. A newly developed technique, which is theoretically able to identify single neuron connections is shown here: See link or DOI: 10.1038/ncomms14162 .

The short answer is no. The DTI scans are at best around a mm volume range in humans and depending on the algorithm used for tracking the changes in the eigenvectors of the diffusion tensor can be confused by multiple tracks within a volume element. The Basser paper listed outlines some of the problems though its a model system for direct neuronal activity. A link to the PMC version is provided below.
