I have found various tools that have been developed to measure structural aspects of integrated care as well as measures assessing healthcare provider’s attitudes toward care teams but have been unable to find a measure specific to integrated care delivery.
Not sure, but see if this is in line with what you're looking for -- called the Level of Integration, designed by my team to measure fidelity to critical aspects of integrated primary care. If this does the trick, I can send you scoring procedure and some psychometric characteristics.

The question you raise is a key theme I focus on in my doctoral thesis. I published a paper entitled "Shared Mental Models of Integrated Care: Aligning Multiple Stakeholder Perspectives" which focuses on the issue of understanding how managers and clinicians perceive and understand integrated care strategies. The framework presented in this paper has been modified recently through consultation with diverse stakeholders in Canada. My next task is to develop a measurement tool based on this framework. James and Olaf: There are definitely some questions within your measures that start hitting on what I am looking for. I was hoping to get a broader scope on type of healthcare providers though. I was also hoping to find items related to how integration is conceptualized and perceived, not only by providers working at an integrated care site but other providers as well.
Jenna: I read your published article which was what started my search into finding a measure assessing exactly what you were talking about. I have been working on my masters thesis and I thought I had a question but there is no measure (that I have found so far) that really captures what I am interested in.

I think attitudes fluctuate and have much greater variance then is indicated in a single measure at a single time. In my dissertation I simulated changes in appraisal of workers and saw dramatic changes in attitude in just a few minutes with our successful manipulation. Work by Hanges and Williams shows similar changes in a performance appraisal setting. You might consider developing your own instrument and utilizing smartphone technology to gather data longitudinally, a method called ecological momentary assessment, or experience sampling. I'm really not sure if this will be of any use or relevance to what I understand you're after, but I've just completed by doctorate and part of this was looking at knowledge perceptions and attitudes of healthcare staff toward the use of Motivational Interviewing with forensic psychiatric patients. Essentially this questionnaire was designed as a training questionnaire but I used it to discern those for whom this approach was perhaps more suited and those for whom it would provide more of a challenge. It draws out the care providers thoughts and ideas about the method by which care should be provided for certain groups, which basically swing beteween the medical model or the client-centred nature of MI. It's called the MIKAT (Motivational Interviewing Knowledge and Attitudes Test) and can be found on the MI website, I believe.
