I suggest you to please read the work of Zane Nolan and Mak Winnie (2003). Let me also suggest you to study the thesis of a master student, which i hope will help you clear about formulating or adopting standardized questionnaire. Thirdly, you can also read the scale (questionnaire) developed by Daya S. Sandhu (1994).

A preliminary literature search using Google Scholar may be helpful. Coping strategies are broad and multiple. You may need to find a measure which is brief to reduce respondent fatigue, but accurately fits your study objectives and population being studied.
