If varicocelle is confirm pain can be indication for surgery i.e varicocelectomy.
If azospermic(nil spermogram) suspect cystic fibrosis with/without absent vas. Chromosomal study is indicated. In absent vas case upper urinary tract evaluation is indicated for renal agenesis .
varicocoele of grade 1 usually does not make any pain and in many cases varicocoelectomy do not relief that pain. The epidiymal cysts are of more concern as they are multiple
please exclude any testicular pathology. grade I VARICOCELE itself is unlikely to cause this amount of pain
