I would like to use and/or adapt a survey/questionnaire used in one study of a particular author. The research paper's full text is published in the institution's repositories and is labeled "available for free and open access".
As stated above, the author can't be reached since no contact details are provided and the institution doesn't keep contact info of their students who graduated years ago.
I see no problem, you cite if the info may be put in a standard format or , otherwise, you provide the link to the free open access text. If the paper has not passed review because the text is just text and not a real publication, you may consider to detail that it is a text that has not gone to peer review. But other than providing all the necessary info I see not problems in referencing the text/paper.

If no prohibition is mentioned in the paper, and you cite the source properly, then there should not be any problem with using the survey.
Indeed, just go ahead and use with a citation; if you do amend the material, make it clear that you have amended it, and how you have amended it.
Check the copyright statement. Most statements are open for Fair Use kike Research and education and some add prohibitive statements. Reading copyright statement saves any problem later.
I disagree with Bushral. The authors' names and the citation statement are facts, and facts are not subject to copyright

A properly cited science creation will always be O K. It is not necessary to contact the author. Just recognize him by citing him.
