To put it in a simple way. If you give the same amount of money to one group of poor people and one group of middle class people then which group is going to have bigger improvement in life quality? In general the high HbA1C group has patients with the worst diabetic control that is why they will have more dramatical change.
Dr. Weinrauch brought up a very important concept about "effect of enrollment". There are several RCT showed that by simply enroll into a RCT the outcome will be better even for the subjects the placebo group due to more attention from the care team. It may also be true that borderline hyperglycemic patient may have more chance to exprience hypoglycemic episodes that will affect the treatment compliance. I feel privileged to have the chance to read the wisdom from Dr. Weinrauch.
