Dear Dr Safer,
we treat some frontotemporal dementia patients with methylene blue. After we've seen encouraging results in one patient (published case report), we are no treating a small series of patients in individual curative triales with a dosage of 180 mg per day.
With respect to side effects, we ocassionally noted diarrhea and in all patients a greenish colouring of urine and feces.
We manufacture methylenblue capsules by ourself (under supervision of a cooperating pharmacist).
We do not measure blood plasma levels.

The chemical origin the phenothiazines (chlorpromazine, ect) is from Methylene blue!
What is considered to be the mechanism of action with regards to the dementia effect?
Are the benefits of MB on malaria and dementia rather more due to its positive effect on the mitochondria?
After all the dementia in Alzheimer's is mitochondrial (at complex IV). Also there appears to be clear mitochondrial influences during malaria (see "Human malarial disease: a consequence of inflammatory cytokine release" http://www.ncbi.nlm.nih.gov/pmc/articles/PMC1629020/ )
