I am looking for a short questionnaire to assess the quality of sleep, ideally it should not have more than 5 questions, but I am having difficulty finding one. Any suggestions ?

You could choose questions that focused on your specific research interests / hypotheses. If it were me, the five questions would concern difficulty falling asleep, sleep disturbance (all cause), amount of sleep, daytime sleepiness, and overall sleep quality.
