We are intending to acquire a licence for an online survey tool at our institute. It will be applied in different research projects for both qualitative and quantitative surveys. It should have no limitation regarding the number of questions, have a decent variety of question types (incl. filter questions), and should allow extracting the data as a CSV or Exel file.
I do really appreciate your suggestions together - best with a short reason why you suggest a particular tool (OR why not). Btw.: Also open source tool suggestions are welcomed.
Benedikt- You can use StatPac or KeyPoint to create a survey for both types of data extrapolation. The drawback with Survey Monkey is that you are limited in terms of how many questions you can ask the respondents.

@Michele: As far as I know are the number of questions in SurveyMonkey only limited in the basic version; it is OK if if the licence costs sth. But of course I will check out StatPac and KeyPoint. :)
Survey Monkey, Qualtrics, and Survey Gizmo all work well. I think all three export to CSV or Excel files, and I know that Survey Monkey exports to SPSS directly as well. I have used Survey Monkey the most so far, but I've been thinking of switching to Survey Gizmo because they offer a few more options right now (such as survey apps for PDAs, Ipads, smart phones, etc.). It is more expensive, but I think they offer more advanced tech support options as well. I think you can get an educational discount with any of those three as well (at least you were able to in the past).
We made good experiences using UniPark (= questback; ESF-Software, see: http://www.unipark.com/1-1-online-survey-software-for-universities-and-students.htm ) in an international research project (questionnaire in different languages) - I think its capabilities are excellent, pricing for universities is reasonable.
Recently I found QuestionStar (or eQuestionnaire), see http://www.questionstar.com/en-us/ . The pricing is similar, I would try it, as well.
Might give Lime survey a look. It is free and open source. I have used it with one of my business for quite some time with no troubles. www.Limesurvey.org
Our school uses REDCap (http://project-redcap.org/) and Qualtrics (http://qualtrics.com/). Both are browser based software solutions for electronic data collection. Data can be exported from REDCap into SPSS, R, SAS, XLS and from Qulatrics to SPSS, XLS and CSV, HTML and TXT. I have not used REDCap but am finishing up a project using Qulatrics. Survey development was intuitive, filter options were available and question types were plentiful. As to open source - both require "partnerships."

They cite some interesting examples like Prolific http://www.Prolific.ac (just for research) or FourEyes http://www.getFourEyes.com (nice interface and unlimited options) in addition to well established (though commercial) software like Qualtrics and SurveyGizmo.
In addition, I came across some papers (cf. here below) using using QuestBack, which proposes solutions tailored for research http://www.unipark.com/en/

I have found Qualtrics surveys to be excellent. They also provide good training resources for researchers and their team respond promptly to any queries.
Survey Monkey - cheap, user friendly and the vast majority of people have come across it in some way so are less sceptical of clicking on a link
At the University of Tennessee we used Qualtrics and REDCap. When Qualtrics doubled their price we evaluated the alternatives and are currently (2019) migrating to QuestionPro. After having used qualtrics, surveymokey, and unipark, we've switched to Questionstar - because it has the power of qualtrics to the price of surveymonkey ;)
Really, I love Questionstar because it's user friendly and intuitive. Moreover it has statistical methods integrated into the tool, so you can run regressions, correlations, crosstabs, t-tests, etc. directly online. Although they a rather standard methods but you can run them directly online and get a good feeling of your data during data collection phase - without having to export it in SPSS or stata.
